近二三十年来，我国城市医生职业处于一种极为尴尬的处境中：医生的（正式）收入不高、来自患者的信任缺乏、人身安全得不到保障、职业形象较为负面，甚至医学专业的招生都出现了困难。总之，医生被认为是一个“不道德”的职业群体，因为他们常常冷漠地对待病人，并且实施诱导需求、过度医疗和“防御性医疗”、收受回扣与红包等。而这种处境必然影响到医生的服务提供，也必然影响到医疗卫生体制的运行。

那么，医生职业的处境究竟如何？在这种处境中，他们又如何向民众提供医疗服务？为什么医生职业处于这种境况中？这与现行的医疗卫生体制有着怎样的关系？又对新医改产生了怎样的影响？基于以上的分析，下一步的医疗卫生体制改革又该如何展开？总而言之，为什么医生在其执业活动中不断地违反职业伦理甚至成为一种常态，从而被认为是一个“不道德”的职业群体？基于在北京的田野调查，本书对上述问题提出了一种职业社会学的解释。

In the past two or three decades, the profession of urban doctors in China has been in an extremely embarrassing situation: doctors' (formal) income is not high, trust from patients is lacking, personal safety is not guaranteed, professional image is relatively negative, and even the enrollment of medical majors has difficulties. In short, doctors are considered an "unethical" professional group because they often treat patients with indifference and practice induced needs, excessive medical treatment and "defensive medical treatment", and accept kickbacks and red envelopes. This situation will inevitably affect the service provision of doctors, and it will inevitably affect the operation of the medical and health system. So, what exactly is the situation of the profession of doctor? In this situation, how can they provide health care to the population? Why is the profession of a doctor in this situation? How does this relate to the current health care system? And what impact has it had on the new health care reform? Based on the above analysis, how should the next reform of the medical and health system be carried out? All in all, why do doctors constantly violate professional ethics in their practice activities and even become a norm, and thus be considered an "unethical" professional group?
