医疗改革是一项高度复杂的工程，当前中国医改仍面临医疗保险可持续难、基本药物政策执行难、公立医疗机构改革难以及医患矛盾之痛、医师多点执业之痛、基层人才流失之痛，这意味着中国医改还有很长的路要走。本书借用Herder-Dorneich结构模型考察各个时期城市医疗服务递送运行机制，剖析政策是如何影响到医疗机构、医生、社保组织、患者等参与者之间的博弈。通过比较制度分析工具进一步解析各参与人的博弈结果是如何影响到后来政策的制定，从而找出影响中国城市医疗现状的各种因素，并对其实际运作提出合理解释，为政策研究者和制定者提供一个分析的视角。

At present, China's medical reform is still facing the difficulty of sustainable medical insurance, the implementation of basic drug policies, the difficulty of reforming public medical institutions, the pain of doctor-patient contradictions, the pain of multi-site practice of doctors, and the pain of grassroots talent drain, which means that China's medical reform still has a long way to go. This book uses the Herder-Dorneich structural model to examine the operation mechanism of urban medical service delivery in various periods, and analyzes how policies affect the game between medical institutions, doctors, social security organizations, patients and other participants. Through the comparative institutional analysis tool, the game results of each participant are further analyzed to affect the subsequent policy formulation, so as to find out the various factors affecting the current situation of urban medical care in China, and put forward reasonable explanations for their actual operation, so as to provide an analytical perspective for policy researchers and makers.（AI翻译）
